Rental Application and Questionnaire. ( Frequently asked questions --scroll down)
Each Applicant Group (4-6 people) needs to do only 2 things.
1. Print off, fillout, and scan/email me the below rental application (link below) ASAP. I am accepting apps until next Tuesday.( May 11) http://www.on-site.com/resources/application.pdf
2. Print out, fillout, and scan/email me the below questionnaire. Please send all applications and questionnaires together in one email to ensure I group them together with the right people.
Additional Questions for applicant. ( for students only)
1. How long do you intend to rent this house? (For example, if you are a sophomore in college would you prefer to stay until you graduate?)
2. Are you capable of paying a security/holding deposit equal to 1 months rent by June 1? Yes / No
3. Will you be available to move in by June 1, 2010? Yes / No
4. Are you staying during the summer months ( June-August) Yes / No
5. If you are NOT staying during Summer Months, you understand rent is still due even though you will be absent from the property. Please Initial__________.
6. Please indicate whether you are comfortable or not with the following "group" lease arrangement.
Group Lease
Tenants all sign one “Group Lease” and are held collectively responsible for the terms of the lease. Late Payments will incur late fees which the group will be responsible for remitting as a whole group. The result is shared risk and shared responsibility.

What next?
I will takea look at the apps/questionnaires and make a decision by Friday May 14th.
I'll contact your group and schedule a meeting where we'll review the lease together, sign the lease, and I'll collect security deposits equal to 1 months rent. This will happen some time between (May 15th- 23rd)
Move in between 24th and June 1.
Rent due June 1.

Information and FAQ's.





I have a 5 Bedroom 2 Bath House Available June 1st.
- The House rents for 1095.00 a month. ( utilities not included)
- 5 Bedrooms, 2 bathrooms ( both have showers and tubs)
- Large Eat in Kitchen.
- Living Room
- Dining Room
- Large Open Foyer.
- Laundry Room on main level.
- Central Heat and air conditioner downstairs, (windows AC units and baseboard heat upstairs)
- Main Bedroom downstairs.
- Gas Heat ( electric upstairs)
- Large Completely renovated front porch
- Front and back yard ( you'll be responsible for mowing the lawn, lawnmower provided)
- What does rent include?
Your bedroom and full use of the house/yard (minus the basement)
- How old is the house?
It was built in 1923. As was the rest of the neighborhood.
- How does the screening process work?
- Once all Applications are in, a scoring system will be used to score each group on credit worthiness, back ground checks etc.
- Decisions will be made within the next week and notification will be sent to each group via email.
- Once the selection has been made, security/holding deposits will be collected and leases signed. ( There will be a sit down meeting with the tenants and myself)
- Do all of the applicants in each group need to fill out and sign their applications for the group to be considered?
Yes, all applicants should fill out their applications and scan/email to keithpolarek@yahoo.com These applications will be checked this week, so send them no later than next Tuesday May 11th. Please also remember to include the other names of your group, so I can make sure to score the right applicants with the right group.
- Is there a security Holding/Deposit and what does it cover?
Yes, it’s equal to one months rent for each tenant or member of the group. People usually split the rent acording to room size. Downstairs bedroom is 260, 3 other upstairs bedrooms are 215 each and the small one upstairs is 190. The purpose of the holding/security deposit is to show your commitment to moving in on or before June 1 as well as a maintenance account to pay for repairs should they be needed as a result of tenant negligence.
- When will it be due?
June 1, 2010
- When would the new tenants move in?
As early as May 24th, until June 1, 2010.
8. How long does the lease cover the group?
12 months
